The MAP sensor will usually cause rough idling, hesitation, and an increased fuel consumption.
At 200,000 km (which is only 125,000 miles: mine has done loads more than that and it still goes
) it is most likely a blocked/disintegrating catalyst.
By all means change the MAP sensor, you can do it without getting under the car, but I very much doubt it will solve the problem.
