It isn't a pressure test you need, but a test for combustion gas in the water.

What happens is the head has a slight warp, and a teeny teeny bit of gas is leaking from a cylinder into the water, due to the very high pressure. A pressure tester from the radiator end will not force water the other way, so it will pass.
Over time this blows the water into the expansion bottle, and when everything cools, leaves air in the top of the radiator.

If you let it cool overnight, and crack the rad cap open, if you hear a little hiss of air escaping the head gasket needs doing, and most likely a slight skim off the head to correct the warpage.

If it suddenly breaks, go back to the last thing that you did before it broke and start looking there :)

It's a cylinder leakage test you need. That's the only way to be 100% sure.
